Emergencies and after hours

 

     If you are experiencing a life threatening emergency, call 911.  If it during office hours, please call, so that we can help best direct you.  As a rule, we discourage walk ins to the office.  We try to give priority to patients who already have scheduled appointment times.  Walk ins will be assessed by our nurses and triaged to be worked into the physician's schedule.  If it is after hours, you can reach the on call physician by dialing our main number (214-368-6341) for Drs. Abel, or (214-543-3200) for Drs. Moore and Roberts.

Prescription Refills

 

     Handled during office hours only

     Most efficiently handled by calling your pharmacy and having them fax us a refill request.  Otherwise call the office to discuss with a nurse.

     For triplicate prescriptions, please call 5-7 days before needed.

 

School and Camp Forms

 

     Please drop by, fax or email to office.  AMRPediatrics@gmail.com

     Please allow for up to 1 week for completion

     Need to have had a physical within the last 12 months

 

Medical Records Request

 

     Please allow for 1 week for completion

     Must request in writing (forms can be found here)

     Processing fee of $25 per child
